Lyndsey – Health and Wellbeing Coach
Hello! My name is Lindsey and I’m the health and well-being coach. My main focus is to support patients to self manage their health by increasing their knowledge, skills and confidence to set health related goals and work towards achieving them. I can help with weight management, mental health and helping to self manage a variety of health conditions . Outside of work I enjoy spending time with my family and going to the gym.
Health and wellbeing coaches predominately use health coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers
Health and wellbeing coaches will:
- Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.
- Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in manging their own health and wellbeing, to live independently, and improve their health outcomes through:
- Providing interventions such as self-management education and peer support; and
- Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual; and
- Working with the social prescribing service to connect them to community-based activities which support their health and wellbeing
- Example – Obese/overweight patients, newly diagnosed high blood pressure, diabetes patients. Known chronic disease patients who are struggling and need motivation. Each patient gets 30 min of F2F appointment and Lyndsey will book any follow ups if required.
- Larger group sessions for patients with similar needs will be planned in the future.

Fill an administrative eConsult, (or call for an eLite only if you have no access to the internet) select “other requests or documents or paperwork” and ask to be booked with the Health and Wellbeing Coach, giving some information of why you would want to see her and best available times (i.e. available Monday mornings if off work)

You will receive a text message from our Care and Admin Coordinators to confirm your appointment date and time.
All initial appointments with our Health and wellbeing coach are face to face.
