Access to your GP Medical Records

Online Access to your GP Medical Record

As from 2nd August 2023 new patients joining the surgery will automatically have full forward access to their GP health record from the date they register with us.

This can be accessed via sign up/login to the NHS App. Patients should not need anything from the surgery as the relevant identity checks can be completed via the NHS App.

If a patient cannot verify their identity through the NHS App, they will be asked to contact the surgery.

In this instance they will need to complete an Online Medical Record Access Form and upload a photo of themselves holding their photographic ID and upload their proof of address.

Use this form to provide required ID documentation evidence to support your application if this has not been possible via the NHS APP You will need to complete a new Online Medical Record Access Form and provide 2 proofs of ID (1 photographic and 1 with proof of address).

    Access to Detailed Coded Medical Records

    Detailed Coded Access is coded information of your health condition or operations. This does not include consultation text written by your GP or Health Professional. If you want to learn more Click Here

    Should patients want access to historical entries in their health record, they will need to complete the Detailed Coded Access form and provide 2 forms of ID. When completing the form they will be asked to upload a photo of themselves holding their photographic ID (The photo must include your face so we can confirm your identity) up to their face and upload their proof of address. Patients that already have past detailed coded record access will still have this facility.

    If  patients want to see their Full medical record this would be a SARS request and they would need to put this in writing
